Description:
Assesses patients and families and assists in discharge planning and coordinating care and patient transition post hospitalization. Adheres to Wheaton Franciscan Healthcare’s performance expectations and performs all defined services and other related duties in accordance with the mission, vision and values of MercyOne.
What you will Do:
Assesses needs.
Coordinates appropriate services and resources and makes referrals.
Intervenes in catastrophic illness situations.
Provides emotional support to families and patients.
Collaborates with other disciplines to meet patient needs.
*Hours/Schedule: *
Full-Time / 40 hours weekly (1.0 FTE)
*Minimum Qualifications: *
Bachelor’s degree in social work
Current Licensed Social Worker in the state of Iowa or must obtain license in Social Work within 18 months of hire.
