Job Description:
A clinically competent, registered nurse who delivers specialized care to the patient in the health care continuum utilizing the nursing process to assess patient health problems and needs, develop and implement nursing care plans, evaluate and implement appropriate interventions and maintain medical records. May advise patients on health maintenance and disease prevention or provide case management. Provides education and counseling to patient and their families regarding diabetes self care. Teaches patients in an individual or group setting on the importance of wellness. Promote healthy lifestyles for diabetic patient to instill in their everyday living such as portion control, healthy food choices and exercise to manage their disease. Participates in projects related to area of specialty. Collaborates with and serves as a resource for other health professionals to promote delivery of quality services and patient education within the organization as well as the community. Effectively coordinates patient care with other health team members, while maintaining standards of professional nursing. Capable of adapting to a changing environment.
Qualifications:
Graduate of an accredited school of nursing
Bachelors of Science in Nursing (BSN) preferred
Prefer current health related experience within the past five (5) years
Prefer minimum of three (3) years experience in pertinent clinical specialty.
Current license as a registered nurse from the State of Iowa
Maintains valid licensure/certification/registration*
Current Diabetes Educator preferred
Illinois License preferred
Will work at both the Burlington and Fort Madison Campus
